What is a Parasomnia?

Parasomnias are disruptive sleep disorders that involve abnormal movements,
behaviors, or emotions during sleep. They may occur in
REM sleep (dream phase)
or NREM sleep (deep/restorative sleep).

Parasomnia Sleep Test with Video PSG

Video PSG (vPSG) is the gold standard test to capture these events, as it combines brain waves,
breathing, muscle tone, and
synchronized video recording.

REM vs NREM Parasomnia

NREM Parasomnias (Occur in deep sleep, mostly first half of the night)

Usually occur in children & young adults, patient often has no memory of the event.

  • Sleepwalking: Walking or performing activities while asleep.

  • Night Terrors: Sudden screaming, intense fear, rapid heartbeat; person often does not recall the episode.

  • Confusional Arousals: Waking up in a confused, disoriented state.

  • Sleep Talking: Verbal outbursts, mumbling, or coherent speech during sleep.

REM Parasomnias (Occur in dream sleep, mostly second half of the night)

More common in older adults, often linked with neurodegenerative conditions (e.g., Parkinson’s). Person usually remembers the dream.

  • REM Sleep Behavior Disorder (RBD): Acting out vivid or violent dreams (kicking, punching, shouting).

  • Nightmares: Vivid dreams causing distress, usually remembered in detail.

  • Sleep Paralysis (sometimes classified as REM-related): Temporary inability to move while falling asleep or waking up.

Who Should Get Tested?

Your pressure requirement (measured in cmH₂O) is identified during the study to keep your airway open. At this
pressure setting, you can clearly see the before-and-after effect—how breathing events such as apneas and
hypopneas drop dramatically and oxygen levels rise compared to the diagnostic phase.

Night Terrors /
Sleepwalking

Frequent screaming,
confusion, or walking in
sleep.

Adults with Acting Out
Dreams (REM Sleep Behavior Disorder)

Punching, kicking, or violent
behaviors during sleep.

Unexplained Nighttime
Movements

Jerks, talking, or strange behaviors
with no recall.

Suspected Epilepsy
vs Parasomnia

Doctors unsure if episodes are
seizures or sleep disorder.

Safety
Concerns

Patients leaving bed/house during
sleep, risk of injury.

Recurrent Sleep Talking
/ Bedwetting

Persistent issues affecting sleep
quality or daily life.

Parasomnia vs Other Sleep Disorders

Disorder Main Symptom Test Needed Why Differentiate?
Parasomnia Abnormal movements/
behaviors in sleep
vPSG with EEG & video Prevents misdiagnosis with epilepsy
Sleep Apnea Breathing pauses, snoring PSG / HST Can trigger parasomnia-like arousals
Epilepsy Seizures, jerks, nocturnal events EEG / vPSG Seizures often confused with parasomnia
